I acknowledge that I have voluntarily chosen to receive a sports massage therapy session from Heal Active, I understand that:

1. Sports massage therapy involves physical manipulation of muscles, tendons, and soft tissues, which may cause temporary discomfort, soreness, bruising, or other minor effects.

2. This massage therapy is not a substitute for medical treatment, diagnosis, or examination. I acknowledge that the therapist is not a licensed medical professional.

3. I am responsible for informing the therapist of any pre-existing medical conditions, injuries, allergies, or sensitivities before treatment.

4. I understand that massage therapy may carry risks, including but not limited to aggravation of existing conditions, and I voluntarily assume those risks.

Release of Liability

I hereby release, waive, and discharge heal active, its therapists, employees, and affiliates from any and all liability, claims, demands, or causes of action that may arise from any injury, loss, or damages related to my participation in this therapy, whether caused by negligence or otherwise.

Medical Disclosure and Consent

I confirm that:

I have consulted my doctor or deemed myself fit to receive sports massage therapy.

I do not have any medical conditions that would prevent me from safely receiving massage therapy.

Cancellation and Refund Policy

I acknowledge that I must provide 24-hour notice for cancellations or rescheduling. Failure to do so may result £20 fee.
